Defects: At Risk, Immediately Dangerous, and NCS

Defects are recorded, categorised and resolved in Manifold, carrying open items forward to an engineer’s next visit.

Defect Categories

  • At Risk (AR): an appliance or installation with a fault that could become dangerous.
  • Immediately Dangerous (ID): a fault dangerous now, requiring immediate action such as a warning notice or disconnection.
  • Not to Current Standards (NCS): installed to an earlier standard and not currently dangerous, but worth recording.
  • Each defect links to an appliance and carries severity and remedial guidance.

Defect Lifecycle

  • Track defects through open, in progress, resolved, and deferred states.
  • Record the action taken, such as a warning notice issued, appliance disconnected, or capped off.
  • Open defects carry forward so the next engineer sees them as on-site context (Fault Memory).
  • Action-required outcomes should drive a remedial plan, not just sit on the record.

Troubleshooting

  • Defect not on the record: confirm it was captured against the appliance before record generation.
  • Carry-forward defect missing: confirm the prior defect was left open rather than resolved.
  • Wrong severity: correct the category before the record is generated, as records are immutable.

Actions Reference

  • Immediately Dangerous: take immediate action, such as issuing a warning notice or, with permission, disconnecting or capping off.
  • At Risk: advise and agree remedial action; record the recommendation clearly.
  • Not to Current Standards: record for awareness; it is not dangerous now.
  • Always record the action taken so the record and any warning notice agree.

Review Before You Generate

Records are immutable once created, so the review step is where quality is enforced. Generate only when the appliance results, defects, photos, and signatures tell a complete and defensible story.

  • Confirm each appliance has the readings its type requires.
  • Confirm defect categories (AR, ID, NCS) and remedial actions are correct.
  • Confirm the engineer signature is present and the certificate identity is right.
  • Resolve anything ambiguous with the engineer before generating, not after.

Why Immutability Helps

A record that cannot be quietly edited after the fact is what makes it trustworthy evidence. Manifold leans into this: generation is idempotent per check, and corrections happen by generating a fresh record from corrected source data where appropriate.

Troubleshooting

  • If a record is wrong, do not look for an edit button; correct the source and regenerate where the workflow allows.
  • If a record pack is missing a certificate, confirm each underlying check has generated its record.
  • If generation is blocked, the check is missing a required field, an appliance result, or a signature.
